AGREEMENT & CONSENT

It is expected that everyone in the Purple Carrots Drama Studio community adheres to the following:

1. Updates to Information
   I agree to inform Purple Carrots Drama Studio Inc. of any changes to the relevant information or within the registration form. I also agree that this form is valid for all current and future services offered by Purple Carrots Drama Studio Inc.
2. Participation Notification
   I acknowledge that I must advise Purple Carrots Drama Studio Inc. in writing if the Participant is unable to participate in the activities and services provided.
3. Liability Waiver
   I agree that, having taken such precautions as in its discretion are deemed advisable, Purple Carrots Drama Studio Inc. will not be held responsible for any injury, sickness, death, or accident to the Participant, or for any loss or damage to personal property resulting from participation in Purple Carrots activities or services.
4. Medical Authorization
   I authorize Purple Carrots Drama Studio Inc. to secure medical care for the Participant on their/my behalf in case of an emergency. If the Participant requires medical attention beyond any first aid furnished by or on behalf of Purple Carrots Drama Studio Inc., I agree to be responsible for any expenses incurred.
5. Release of Claims
   I release Purple Carrots Drama Studio Inc. from all claims and liabilities whatsoever arising from participation in or attending Purple Carrots Drama Studio Inc. programming by the undersigned, the undersigned’s child(ren), or any associated spectator.
6. Anti-Harassment and Discrimination Policy
   I agree to abide by the Purple Carrots Drama Studio Inc. Anti-Harassment and Discrimination Policy as stated on their website: https://purplecarrots.ca/anti-discrimination-harassment-abuse-policy/
7. Financial Responsibility
   I agree to assume full responsibility for payment to Purple Carrots Drama Studio Inc. and will abide by the payment instructions stated on Purple Carrots financial statements and invoices. Payments can be made via:
   • E-Transfer: payments@purplecarrots.ca
   • Cheque: Payee “Purple Carrots Drama Studio Inc.”
   • Online using a credit card or debit cardIf financial assistance is required, I will notify Purple Carrots, and a confidential arrangement will be made. Registration for Purple Carrots Drama Studio Inc. is not confirmed until payment is received.
8. Cancellation Policy
   • From the date of registration up to 3 weeks (or more) before the program start date: 50% Purple Carrots Drama Studio Inc. credit.
   • 2 weeks (or less) before the program start date: No credit.
   • Credits expire within one year and cannot be exchanged for cash.
   • No cash refunds are provided—only Purple Carrots Drama Studio Inc. credit.
9. Personal Support Worker (PSW) Policy
   Purple Carrots recognizes the important role personal support workers (PSWs) play in our participants’ lives. Participants have the right to accommodations, such as PSWs, to support their success in our programs. When a PSW is engaged:
   • The PSW’s primary responsibility is to provide personal support to the Participant (referred to as the “Carrot”), ensuring their safety, comfort, and well-being during the workshop.
   • The PSW should encourage independence in the Participant they are supporting and assist personally as needed.
   • The PSW is not responsible for any administrative, facilitating, or operational duties related to the workshop.
   • The PSW must maintain a professional and respectful attitude towards the Participant and other participants in the workshop.
   • The PSW is expected to adhere to any confidentiality agreements regarding personal information shared during the workshop.
   • The PSW is not an employee or representative of Purple Carrots and is solely responsible for supporting the Participant in accordance with their individual needs.
   • The PSW is an independent contractor and not an employee of Purple Carrots. They will not represent Purple Carrots in any capacity and are not entitled to any benefits or compensation beyond the terms agreed upon with the Participant or their guardian.

     Compensation
     The PSW will not be compensated by Purple Carrots Drama Studio Inc.

     Confidentiality and Privacy
     The PSW agrees to maintain the confidentiality of all personal, medical, and sensitive information regarding the Carrot. This includes respecting privacy laws and the ethical standards surrounding the treatment of the Carrot’s personal information.

     Liability and Insurance
     The PSW is responsible for ensuring they have adequate liability insurance in place (whether through an employer or independently) if necessary, to cover any incidents or injuries that may arise while supporting the Carrot. Purple Carrots is not liable for any claims related to the PSW’s actions during the course of the workshop.

   Purple Carrots Drama Studio can provide 1:1 support in workshops at an additional cost, if needed.
